The Diablo backstory that sets up Lord of Hatred
The recap retraces Sanctuary's origin, Lilith's return, and how Mephisto carries the story into Lord of Hatred.

If you are stepping into Diablo 4 for the first time, the series’ history can feel like a wall of demons, angels, and very bad decisions. The useful version is much shorter. Sanctuary was born from an angel and a demon, Lilith and Inarius split apart, and Mephisto never really stopped shaping what came next.
The story before Diablo 4
The earlier games do a lot of heavy lifting, but the key beats are simple. Diablo 1 ended with Diablo defeated and a hero making the worst possible choice by putting the Soulstone in his own head. Diablo 2 brought Diablo back with Mephisto and Baal, then shattered the Worldstone. Diablo 3 pulled angels into the fight, pushed the world to the edge, and showed that humanity and the Nephalem could stand on their own.
That is the foundation Diablo 4 builds on. The rest is just details that matter less than the pattern.
Where Diablo 4 picks up
Lilith, the daughter of Mephisto, and Inarius created Sanctuary together. Their partnership collapsed, Lilith was cast into the void, and when she returned in Diablo 4 she tried to force humanity to grow stronger before the next disaster arrived. The idea had a point. Her methods did not. By the end of the base game, Lilith was dead, Inarius was dead, and Mephisto was still working in the background.
That is the real threat. Mephisto does not need to be loud to be dangerous. He shows up as a wolf, pushes people toward bad choices, and keeps playing the long game. When the base campaign ended, Neyrelle trapped him in a Soulstone and left to deal with it herself, but that was never a clean solution.
What Vessel of Hatred changed
In Vessel of Hatred, Neyrelle loses control fast. The Soulstone starts corrupting her, Mephisto keeps pressing from inside it, and the journey into Nahantu goes from bad to worse. Allies die, trust breaks, and a deal meant to save the region opens the door for Mephisto to break free.
Once he is loose, he does not stay hidden for long. Mephisto takes over the body of Akarat, the long-dead prophet whose teachings helped define what humanity believes. From there, he can move in plain sight as the second coming of Akarat while heading toward Skovos, the home of the Firstborn, the first generation of Nephalem.
Why Lord of Hatred starts there
