
Escape the Backrooms will add a new place to get lost in when the Plastic Mariana update arrives on August 27, 2026. Level 421 leads from an abandoned M.E.G. facility into a children’s hospital connected to Bruce’s Ball Pit, which sounds like exactly the kind of combination that should come with several warning signs.
The developer announced the update in its Steam announcement, describing Level 421 as a long stretch of polymer that players must push through before escaping into Overgrowth. It will be the first new level added since the game’s 1.0 launch in October.
Level 421 connects a hospital to Bruce’s Ball Pit
The new area pairs sterile hospital corridors with brightly colored children’s spaces, creating a particularly strange contrast for a Backrooms level. One section features a shark figure behind blue bars, while other areas descend into darker corridors where a player in a yellow protective suit faces a grinning creature.

A new song will accompany the nightmare
The update will also bring a new rendition of the game’s main Backrooms motif. Norwegian producer and DJ Vairo worked with the team on Wanderer, a terrifying track that recreates and uses Ins0mic‘s motif with permission. The full song is available on Spotify.
The August update will include more than the new level. Bug fixes will eliminate softlocks on Nintendo Switch 2 and address some multiplayer connectivity issues, giving groups a better chance of reaching Overgrowth without getting trapped by a technical problem.
